Rates
I am an in-network provider for most Blue Cross Blue Shield PPO, HMO, and Blue Connect plans. I am also an in-network provider for UnitedHealthcare, Cigna and Aetna. The use of out-of-network benefits may be an option depending on your plan. You are responsible for your co-pay and/or deductible; payment is due at the time of your visit. For those individuals who either do not have insurance or choose not to utilize their benefits a sliding scale is also available. If further information is needed, please contact me by phone. Benefits and/or rate information will not be provided by email.
Those individuals interested in supervision for licensure may either call or email to inquire about the fee.
Payment
Cash, check or credit card are accepted for payment.
Cancellation Policy
If you do not show up for your scheduled appointment, and you have not notified me at least 24 hours in advance, you will be required to pay the cost of the session unless such cancellation is due to illness or an emergency.
